DIV 6 REQUESTED RETURN OF DONATION
ROUTING FORM
The form is for requested return of donations of less than $250 or equivalent as per HCO PL ******
1. RECEPTIONIST
Initiated the Routing Form.
2. CHAPLAIN
1. Interviews the person to find out why they are requesting a return of their donation.
Handle what comes up to salvage them if possible
2. If they insist on getting their donation back, have them make out a written request that gives the following information:
Their full name and address
The amount being requested returned
What Service it is for
When the donation was made
Whether they took the service (if it was more than 3 months earlier, they are ineligible, show them the policy) and whether they completed it.
3. Have the person sign a release and waiver that in exchange for return of their donation they release and hold the church and its principals harmless. Make a copy and attach it to this routing form and send the original to Val Docs.
4. Inform the person that they will no longer be eligible for services in any Church of Scientology.
3. FBO
1. Verify that the Church actually received the funds from the person (the check or credit card cleared if not a cash payment).
2. If the payment is so recent it has not yet been banked, return the check, cash or recredit his credit card on the spot and have Treasury write a minus invoice.
3. If the amount has already been banked, transfer the funds from the FO #1 account to the Main Account.
4. TREASURY SEC
1. Verify the earlier steps on the routing form are done, including checking that the funds were in fact received by the Church.
2. Write the check, hand it to the person and get a receipt.
3. Send this Routing Form to CF/Addo.
5. CENTRAL FILES OFF
1. Take the necessary actions to deadfile the person as covered in HCO PL 7 Jun 65 Entheta Letters and the Dead File, Handling Of.
