ENROLLMENT FORMS AND WAIVERS
The most major departure from the Ideal Scene is the breakdown at OSA Int. The Legal Safeguards area could not find copies of all the enrollment forms being used. They could not even access the forms on the CD's they had sent out to the orgs.
There have been no programs on this subject since 2000 (when all the enrollment forms and waivers were re-done and sent out to all orgs following the Clearwater case). According to Legal Safeguards Chf OSA Int the bugs on the reprinting of the forms are that the orgs don't allocate funds to them and they are not covered in the Int level FP either and so supplies of the forms run out.
The Forms themselves are designed so that they have individual org names on the forms instead of "Church of Scientology of _____" and they were designed with foil so they are expensive to produce.

- Enrollment form for services
- PC folder
- Confidential materials
- Introspection Rundown
- Orientation
There is no standardized release, waiver and confidentiality agreement for people leaving staff or the Sea Org.
